I'm confident the problem is with the form not the template. It has to do with the way Mediawiki interprets any text beginning with "http(s)//". I've tried putting 'nowiki' tags and quotes around the field value with interesting results but not what is required. - Yeah. It's a software bug/incompatability with Form:WpPage introduced somewhere. The first thing I did was to check you had the latest version of semantic forms.
