'Document Provenance' information. The template should be the first item on Document pages. See also template:FileProv.
The blue parameters not be marked up at all; they should be Wikispooks page names (whether or not the page yet exists).
Usage:

|Author=The name of the author. For a pseudonym, use single quotes. If anonymous, use Unknown
|ContentsTitle=Mainly for backward FileProv compatibility but useable for new Documents too
