The diagrams in this presentation provide a clear layman's understanding of both the Fukishima plant and its breakdown following the earthquake. The most worrying part of the presentation is provided by the last but one page illustrating the failure to maintain the water supply to the spent fuel storage ponds and consequent release of intensely radio-active fission products to the atmosphere.
The document is written from the perspective of the nuclear industry, with all the caveats that implies. It is nonetheless provides very useful information in a clear and understandable format.
